Department of Prevention and Treatment of Emergency Conditions

Department of acute myocardial infarction was founded in 1991 on the basis of the intensive care unit of the Institute of Therapy on the initiative of the head of the Institute, Academician L.T. Malaya – the first supervisor of the Institute. Since 2005 the department is headed by Doctor of Medical Sciences, Honored Doctor of Ukraine (1998), member of the European Society of Cardiology Kopytsya N.P.

Objectives of the department:

- study of pathogenic mechanisms of reperfusion injury and postinfarction myocardial remodeling in patients with acute myocardial infarction with ST segment elevation (STEMI) after primary percutaneous coronary intervention (PCI) and evaluation of the cardioprotective effect of early administration of platelet P2Y12 receptors inhibitors  by:

- study of the role of inflammatory markers (expression of the genes of the inhibitor of macrophage migration factor (MIF), TNF-α, IL-1β level), vascular lesion (expression of vasculoendothelial growth factor-A (VEGFA) genes in mechanisms of reperfusion injury after primary PCI in patients with STEMI;

- evaluation of the cardioprotective effect of early (prehospital) administration of platelet P2Y12 receptors inhibitors (brilinta, clopidogrel) before the primary PCI in patients with STEMI and the effect of this therapy on the expression of MIF, TNF-α, VEGFA and inflammation markers (SRP, IL-1β);

- study of the effect of reperfusion on the functional state of the endothelium (level of endothelial NO synthase, endothelin-1) and evaluation of endothelium-protective effect of early administration of platelet P2Y12 receptors inhibitors;

- study of structural and functional changes in the myocardium after the early administration of platelet P2Y12 receptors inhibitors (brilinta, clopidogrel) and primary PCI in patients with STEMI, based on modern technology of ultrasound examination of the heart - determination of strain, strain rate of cardiac muscle and parameters mechanical dyssynchrony;

- evaluation of the role of markers of endothelial damage, inflammatory processes and vascular lesion in mechanisms of postinfarction remodeling after primary PCI in patients with STEMI.

Functions of the department:

- organization of scientific researches according to the choose topics;

- conducting a thorough analysis of the results of the research;

- publication of research results in professional journals;

- announcement of scientific research results at conferences, symposiums and congresses;

- registration of patents based on the results of studies;

- introduction of scientific developments in the practice of health care;

- carrying out medical work in the intensive care unit and consultations at the clinic of the Institute;

- participation of the department staff in conducting health education for the prevention of acute myocardial infarction and sudden cardiac death;

The main scientific areas:

- fundamental study of the pathogenic mechanisms of acute coronary syndrome;

- definition of the role of modern biomarkers in the diagnosis, risk stratification and choice of treatment strategy for acute coronary syndrome;

- development and wide introduction in practical public health system of pharmaco-invasive strategy for treatment of acute myocardial infarction;

- determination of highly sensitive electrophysiological markers of sudden cardiac death;

- development of personalized therapeutic schemes for prevention arrhythmic complications of acute coronary syndrome;

The most important research results:

  1. We identified pathogenic predictors of myocardial damage, which lead to progressive heart failure, ventricular arrhythmias, sudden cardiac death, developed prognostic criteria of damage and methods of drug correction.
  2. A new scientific substantiation of the role of inflammatory markers in the pathogenesis of acute coronary syndrome was proposed.
  3. The interrelation of the main mechanisms of the coronary syndrome, such as indicators of necrosis, inflammation and coagulation, is established.
  4. It has been proved that the B-type natriuretic peptide and ST2 are early markers of heart failure in patients with myocardial infarction.
  5. The pathogenic role of vegetative function disorders in predicting of life-threatening arrhythmias after myocardial infarction is determined.

Cardiac arrhythmia research laboratory was created on the basis of the department, which is equipped with an ultrasound diagnostic device, a stress system with an electrocardiograph "Cardiolab SE", Holter ECG monitoring devices with determination  of turbulence of the heart rhythm, rheographic complex with the ability to assess endothelial function.

The program for assessing the slowing heart rhythm (sensitive index of vegetative function) was developed and implemented by collaboration with the staff of “KhAI-MEDICA”.

The clinical base of the department is the intensive care unit (headed by PhD Abolmasov A.N.), which has modern equipment: systems for monitoring of patients, defibrillators, devices for artificial ventilation, electrocardiographs.

Since 1993, under the auspices of the European Society of Cardiology, the department staff participated in multicenter clinical trials - EMIP-FR, BIRD, ZOF-07 HERO-2, EXTRACT-TIMI, EPHESUS, ATLAS, ASCEND, ROCKET, IMROVE, ENGAGE, APEX, etc.

The results of the department's scientific activity are 4 monographs, more than 420 published scientific articles, 16 received patents of Ukraine. The department has 3 doctoral and 16 candidate dissertations.
