Department of prevention and treatment of kidney diseases in comorbidity conditions

The Department of Nephrology was founded in 1992 by order of Lyubov Malaya, director of the Institute of Therapy, No. 195 from 16.09.1992.

20.10.1992 the head of the department was appointed Doctor of Medical Sciences, Professor Ivan Topchy, who heads the department at  present time.

In the department of nephrology, there are 3 candidates of medical science, 4 researchers: senior research workers Semenovykh P.S., Kirienko A.N., research workers Shcherban T.D. and Yakimenko Y.S.

Currently, the department develops and improves medical technologies for diagnosis, prevention and treatment of kidney diseases. Metabolic, immune and mineral disorders in various forms of renal pathology, particularly in diabetic, gouty and hypertensive nephropathy, as well as in chronic glomerulonephritis, are actively studied. Mechanisms of formation of nephrogenic hypertension and accelerated development of cardiovascular complications in patients with chronic kidney disease are investigated.

The results of scientific research are published in monographs, textbooks, articles, methodological recommendations.

The staff of the Nephrology Department constantly takes part in the work of scientific and practical conferences, congresses of the state and international levels, scientific conventions.

The department is the center of the Pharmaceutical Committee of Ukraine on the clinical drug testing in nephrology.

The main directions of the scientific work of the department:

- development of methods for prevention and treatment of cardiovascular complications in patients with chronic kidney disease, taking into account the characteristics of the genetic and immune profile;

- study of the peculiarities of mineral homeostasis in comorbidity of kidney pathology and diabetes mellitus;

- study of the kidneys functional state disorders on the basis of determining the glomerular filtration rate, microalbuminuria and proteinuria levels and renal blood flow parameters at different stages of chronic kidney disease;

- determination of the relationship between the components of the renin-angiotensin-aldosterone system in renal diseases;

- study of changes in central and peripheral hemodynamics, principles and types of remodeling of the cardiovascular system in patients with chronic kidney disease;

- determination of the effectiveness of modern medicines with nephroprotective action in the treatment of chronic kidney disease and development of methods for combined nephroprotective therapy in patients of this category, taking into account the factors of disease progression;

- study of the efficacy and safety of drugs in patients with chronic kidney disease in controlled clinical trials.

The most important results of scientific research:

- specific features of cellular and humoral immunity disorders which contribute to the progression of nephrosclerosis, chronic renal failure and early development of cardiovascular complications are determined;

- the relationship between the severity of fibrotic processes in the kidneys, the degree of renal hemodynamics and the state of the renin-angiotensin-aldosterone system;

 - modern methods of diagnosing cardiovascular complications in patients with chronic kidney disease, taking into account the state of the vascular endothelium is studied;

 - a method for correcting the disorders of mineral homeostasis in patients with diabetic nephropathy is proposed, which provides additional cardio- and nephroprotective effects that allow optimizing treatment in general and inhibiting further progression of the disease;

 - methodological recommendations on the diagnosis and correction of violations of phosphorus-calcium metabolism in patients with chronic kidney disease are developed and introduced.

Employees of the department Semenovykh P.S. and Shcherban T.D. evaluating blood bicarbonate levels in patients with chronic renal failure.

The results of the research were reported at 42 congresses, conventions and conferences of Ukraine, European and international congresses.

The staff of the department published 1 monograph, 2 teaching aids, over 690 scientific articles in scientific medical journals, 14 methodological recommendations were issued, 33 copyright certificates and patents were obtained, 11 candidate and 4 doctoral dissertations were unheld for the years of the department's existence.

Currently, on the basis of the department, one candidate's thesis is carried out.

For active scientific work, the Nephrology Department was awarded a diploma of the Presidium of the National Academy of Medical Sciences of Ukraine for scientific work, which was recognized as the best in 2011.

Head of the department I.I. Topchy - Laureate of the O.G. Zubarev Kharkiv Komsomol Prize (1983) and the P.I. Shatilov Kharkiv Regional Scientific Medical Society (1991), was awarded with the diplomas of the Exhibition of Economic Achievements of the Ukrainian SSR (1989, 1991) and a medal of the National Academy of Medical Sciences of Ukraine (2013).

Employee of the department Semenovykh P.S. was awarded a diploma of the third degree of the "Inventors' Contest 2005" for success in inventing work on the development of new ways of diagnosing and treating kidneys in patients with diabetes mellitus, as well as a diploma of the winner of the IV regional competition "Best Young Scientist of Ukraine" in 2009.
